NEW California Requirements: 

Before volunteering as a Field Trip Chaperone, you must complete the following:

1. Fingerprinting (Live Scan)
If you have not already been fingerprinted and cleared for MH field trips, get a Live Scan form from the school office and visit any Live Scan facility. Once completed, fingerprint clearance lasts through your child’s 8th grade year.

2. Mandated Reporter Training
Create a free account and complete the volunteer training. Please provide your certificate of completion to the school office.

3. TB Clearance / Risk Assessment
Have your medical provider complete the TB Clearance or Risk Assessment Form and return it to the school office.
